Ecclesiastical Marriage

On this page, you will find the marriage form. This marriage form is always required when a future couple wishes to marry in the Roman Catholic Church. The form must be completed digitally.

The form is completed by the pastor himself or by the delegated priest or deacon who will act as a church witness during the wed­ding ceremony. Only the ordained minister in question can submit this form to the diocese. The epis­co­pal delegate then examines the application and grants per­mission or dispensation on behalf of the bishop. Only with such per­mission or dispensation can the marriage be lawfully and/or validly con­clu­ded.

We have a proces­sing time of 6 weeks. If the do­cu­ments are needed sooner, please contact the diocese by telephone.

General information for the ordained minister comple­ting the form:

1. Complete the form entirely digitally. Only the signatures need to be added by hand after comple­ting the form. To do this, you must print the form. After sig­ning, scan the forms and send them to huwe­lijk@bisdomhaarlem-amster­dam.nl. Keep the original form for yourself
2. The form can be completed during a mee­ting with the future couple.
3. The form can be saved in the interim, either partially completed or completed.
4. The form includes an appendix contai­ning all mentioned sources and canons, which can be kept alongside the form. This appendix serves as an aid and does not need to be sent along.
5. You will receive the do­cu­ments supple­mented by the ecclesiastical court back (by post and by email). You can forward/process the original form along with the do­cu­ments supple­mented by the ecclesiastical court.

The diocese will make a digital copy of everything for archi­ving.
